FiO2: 50, bzw 100% bei Hypoxie <80%

meist Typ 2 Diabetiker, meist schleichender Beginn
Therapie:
- cave: BZ-Abfall= Gegahr eines Hirnödems
- Volumengabe!, dann meist keine Insulingabe nötig
pushDose Pressors
- Noradrenalin : Dosierung, typische Indikation
1mg NA mit 100ml NaCl 0,9%
--> 10mikrogramm/ml
davon 10ml abziehen
dann 1-3ml Boli ( = 10-30mikrogr<mm)

- 1-3ml ( entsprechen 10-30mikrogram)
Indikation:
- symptomatische Bradykardie
- Anaphylaxie mit Schock trotz i.m. Gabe
Wirkung:
periphere Vasokonstriktion, HF steigt, pos Inotropie
Krampfanfall

Midazolam 5 (<50kg) bis max 10mg i.v. oder i.m.
Lorazepam max 4mg i.v.
2. Antikonvulsiva
Levetiracetam oder Valproat
bei Schwangeren zusätzlich 16-24mmol Mg i.v. als KI (v.a. Eklampsie)
Hyperkaliämie ( MODERAT BIS SCHWER)
- medikamentöse Therapie
5-5,9mmol/l : milde Hyperkaliämie
6mmol/l moderat - schwere Hyperkaliämie
bei EKG Veränderungen ( QRS Verbreiterung, Bradykardie)
Benzodiazepinintox
Flumazenil
0,5- 1mg i.v
( 5 mg Ampulle! Cave: Senkt Krampfschwelle)
Opioidintox
Naloxon
0,4-2mg
Asthma, akute Exazerbation
- Therapie
1) O2 Gabe
2) Inhalation : S ( 2,5mg alle1 10-15min) +A ( Ipratropiumbromid 0,25-0,5mg alle 30min)
3) bei O2 Bedarf: Steroid, zBT Prednisolon 50mg - 100mg p.o./ i.v.
4) kausale Ursache? ( Infekt, LAE, Allergie)
Eskalation: MG ca 8mmol i.v. als KI , NIV

STEMI-Definition = ST-Hebungen in ≥2 benachbarten Ableitungen ≥1mm, außer:
1) konkordante ST Hebung

IsolierteST-Hebung in aVR(evtl. auch V1)mitausgeprägten ST-Senkungenin mind. 6(-8) Ableitungen, oftkritisch krankwirkende Pat.:
